• Suspend your contributionsUse this form to request a savings suspension from mysuper.
• Change your contributions Use this form to increase or decrease your mysuper contribution rate.
• Make a voluntary contributionUse this form to make a voluntary contribution to your mysuper account at any time.
• Change your account structure Use this form to change your account structure to Minimum Locked or 100% Locked.
• Transfer unlocked retirement savings into your locked account Use this form to move unlocked retirement savings to your existing locked mysuper account.
• Take a babybreak from your fees Use this form to apply for a break from paying your annual fund charges before you go on parental leave.
• Change your investment fund Log into your mysuper account to change your investment fund selection.
• Change your personal details Log into your mysuper account to change your personal details like your address, phone number, email address or beneficiaries.
